Student Response: To complete this exam I read related chapters on Bates and watched videos (visual
guide). I read the patient chart. I then asked the patient questions to gather subjective information and then I
performed the physical examination to gather objective information.
Explain the clinical reasoning behind your decisions and tasks.
Student Response: Tina's symptoms are consistent with GERD diagnosis. To build the plan of care I
proposed with several lifestyle modifications that are known to improve GERD symptoms she is currently
experiencing. I added the scheduled H2 antagonist because antacid alone was not effective and I also
proposed a plan B to escalate treatment to a PPI in case Zantac also fails.

Student Response: One's performance can always be improved. I do feel increasingly more comfortable in
asking adequate questions to obtain information regarding HPI, as well as all the other necessary things such
as current medications, social and family history. If feels as though the process is coming together and my
brain is better able to make sense of it and do things more automatically. Shadowhealth is an
wonder/amazing resource to learn these skills.
Assessment skills are always of great value to clinical practice. Asking Tina Jones these focused GI
questions has made me more aware of questions I should be asking real life patients when they present with
similar symptoms.
