Family Nurse Practitioner Board review questions
1. A 28-year-old pregnant patient gives a history of smoking 1 pack of cigarettes per
day (1PPD). The
... [Show More] nurse practitioner is accurate when she tells the patient that cigarette
smoking is associated with:
A.a macrosomic fetus.
B.fetal hypoglycemia in the first 24 hours of
life.
C.a small-for-gestational age fetus.
D.increased incidence of fetal cardiac anomalies.
2. A patient has developed a blood clot in his lower extremity and is receiving warfarin
(Coumadin®) therapy. Which outpatient laboratory measurement best measures the therapeutic
effect of warfarin?
A.International normalized ratio
B.Activated clotting time
C.PT
D.PTT
3. An expected finding in a patient with long standing, poorly controlled hypertension is:
A.Curley B lines.
B.atrial fibrillation (AF).
C.hyperthyroidism.
D.a laterally displaced apical impulse.
A laterally displaced apical impulse suggests cardiomegaly, a common consequence of
poorly controlled hypertension. The apical impulse, or point of maximal impulse (PMI),
describes the outward movement of the apex of the heart with each beat. It is normally 10
cm from the midsternal line at the 4th or 5th intercostal space at the mid-clavicular line.
Long standing, poorly controlled hypertension results in an enlarged left ventricle. This is
usually described on chest xray or EKG as "left ventricular hypertrophy". Curley B lines
are associated with pulmonary vascular congestion. Atrial fibrillation can be the result of
many abnormalities, but, is not specific to hypertension. Hyperthyroidism is a risk factor
for CHF, but, not an expected finding.
4. A female patient asks "How do I calculate my ideal body weight?" The nurse practitioner
appropriately answers:
A.Start with 100 pounds, and add 5 pounds for every inch in height over five feet.
B.Add 6 pounds for every inch over 5 feet, then add 100 pounds.
C.Start with 100 pounds, and add 10 pounds for every inch in height over five feet.
D.Multiply your height in inches times 2.
The calculation of ideal body weight (IBW) begins with 100 lbs for the first 5 feet in
height. Then, females add 5 lbs for every inch over 5 feet and males add 6 lbs for every
inch over 5 feet.
5. A 21-year old female patient presents with a painful ulcerated vulval lesion. She has multiple
sexual partners. Which of the following is the most likely diagnosis?
A.Syphilis
B.Human papilloma
virus
C.herpes simplex
D.Chlamydia
6. The nurse practitioner is initiating levothyroxine (Synthroid) for primary hypothyroidism in a
71 year old female. The usual starting dose of levothyroxine is 50 (0.05 mg) to 100 mcg (0.100
mg) per day. What would be the most appropriate initial therapy in this patient?
A.25 mcg (0.025 mg) per day
B.50 mcg (0.05 mg) per day
C.75 mcg (0.075 mg) per day
D.100 mcg (0.100 mg) per day
A principle of pharmacotherapeutics in the elderly is "start low and go slow." In older
patients, 25 mcg per day is the usual starting dose. Larger doses may precipitate tachycardia
and chest pain.
7. A nurse practitioner has been sued by a patient for malpractice. The nurse practitioner is
considered to be a:
A.defendant.
B.plaintiff.
C.judicial
claimant.
D.litigator.
8. Before initiating an HMG-CoA-reductase inhibitor for hyperlipidemia, the nurse practitioner
orders liver function studies. The patient's aminotransferase (ALT) is elevated. What laboratory
test(s) should be ordered?
A.Serologic markers for hepatitis
B.Serum bilirubin
C.Serum cholesterol with HDL and
LDL
D.A liver biopsy
9. Which of the following is NOT a symptom of irritable bowel syndrome?
A.Painful diarrhea
B.Painful constipation
C.Cramping an [Show Less]
