Question 1
One way to take responsibility for your health is to formulate a wellness plan. Write four wellness goals for
maintaining good reproductive health. Categorize each goal as short-term or long-term, and explain why
the goal fits the category. (12 points)
1. Not getting an STD: This goal is a long-term one and whether I decide to participate in sexual
intercourse or not, I want to be at least educated enough to know how to protect my reproductive health.
2. Being more aware about my health with doctor appointments: This goal is long-term one because it is
important to know how my health is on a more normal basis then currently.
3. Exercise: This goal is a long-term goal because I want to be able keep my organs in good health, and
exercise is a good lifestyle to live anyways. There are so many positive outcomes from exercise,
especially for your reproductive health.
4. Eating good nutrition: This goal is a long-term goal because eating well also affects how your organs
function and what nutrients are provided to them. I always want to have good reproductive health so I
need to eat right.
Question 2
Do research to complete the table. Locate a state- or county-level governmental organization that
provides information on reproductive health. Use the table to describe how others could access its
information. Then locate a urologist and obstetrician-gynecologist and provide contact information and a
description of at least one service provided by each.
Describe how you accessed this information (for example, an Internet search, phone call, in-person
meeting, or other). Give at least one detail about your research process that might help others who are
trying to find similar information. (8 points)
