3. How do we
monitor patients

4. Which settings
may we find
telemetry being
used in nursing?
5. Which staff
members are appropriate to monitor and care
for patients using telemetry?
6. What is the desired outcome
with telemetry
monitoring?
7. What is the nurses role in caring for clients undergoing telemetry monitoring?
8.
SA node, AV node, bundle of His, right and left bundle
branches, Purkinje fibers
Used to monitor patients with suspected or confirmed
cardiac abnormalities
Electrodes are placed in certain areas of the patient's
chest in order to detect and transmit the electrical activity
of the heart which is converted by the cardiac monitor into
an ECG tracing.
-Tracings are viewed by specially trained health care
personnel & evaluated for cardiac rhythm changes or
abnormalities
Most commonly utilized in acute care inpatient and outpatient clinical setting
Nurses, physicians and ECG technicians who have undergone training on ECG interpretation may participate in
the care of the patient undergoing telemetry monitoring,
however the assessment should NOT be delegated to
non-licensed staff
That cardiac rhythm changes or abnormalities will be
promptly identified & treated in order to prevent hemodynamic compromise or other adverse effects
-Nurses (or ECG technician) may be employed to watch
cardiac monitors and promptly alert the responsible nurse
when an abnormality is seen
-The nurse caring for the patient is responsible for assessing the patient once they are either notified or recognize
themselves any cardiac abnormalities

12. Examples of
Telemetry MonUsed to describe patients who definitely require cardiac
monitoring
Example: Patient with acute heart failure
Describes individuals who may benefit from monitoring
Example: Patient recovering from an angiography
Describes patients who are not candidates for monitoring
because their risk for a significant cardiac event is low
Example: patients with permanent, rate-controlled atrial
fibriliation)
-Cardiac or stroke signs/symptoms (chest pain, syncope,
or new onset arrhythmia)
-Risk for life-threatening arrhythmia (V-tach, long QT interval)
-Intermediate or high-risk for acute coronary syndrome
(ACS)
-Documented myocardial infarction (MI)
-Recent CPR
-Hemodynamic instability
-Moderate or severely imbalanced magnesium or potassium level
-Drug overdose (suspected or confirmed)
-Bedside procedures requiring conscious sedation
-Continuous administration of vasoactive medications
