C. PHYSICAL EXAMINATIONS
General Assessment:
Area Normal Abnormal Findings
Skin, Scalp, and Nails Skin is smooth and even without any lesions and masses. Her skin has a warm temperature, has a good mobility and turgor with no edema detected.
On the hair and scalp, our client has a natural black hair color and her scalp is clean and dry with no sign of any hair loss/ hair patches.
Nails are cleaned and manicured. Pink tones is also visible and has a normal degree angle between the nail base and the skin. Moreover, the nails are hard, smooth, firm and pink tone returns immediately to blanched nail beds when pressure is released. No abnormal findings detected
Head and Face Head is still, upright, hard and smooth without the presence of any lesions.
The client’s face is in symmetry with a round appearance and no abnormal movements observed.
Temporal artery is elastic and not tender. Also, the temporomandibular joint has no swelling, tenderness,
or crepitation with movement. Mouth opens
and closes fully No abnormal findings
Eyes Client has a normal near visual acuity with normal peripheral vision.
On the corneal light reflex test, the client’s corneas are in exact same spot on each eye and on the cover test, the findings shows that both eyes remains fixed straight ahead after being uncovered.
Furthermore, the position test results are smooth and symmetric eye movement thought all six directions.
The external eye structures show a normal findings wherein skin on both eyelids has no redness/lesion and the client can easily close lids. Eyeballs are in symmetry and the conjunctivas are clear and moist with no abnormal discolorations, swelling and lesions
Lacrimal apparatus has no swelling and drainage noted.
Cornea is transparent and lens is free of opacities
Iris and pupil is round, flat, normal in size and constrict in response to light also there is a red reflex. The client has an impaired vision called myopia wherein the test results 20/30 visual acuity
Ears The color of the client’s ear is the same with the entire face, it is symmetrical and the position of the auricle are aligned with the outer canthus of the eye for about 10 degree from vertical.
Furthermore, the era is mobile firm and non-tender. The pinna recoils after is folded.

Nose Color is the same as the rest of the face; the
nasal structure is smooth and symmetric; the

Mouth Lips are smooth and moist without lesions or swelling.
Thirty-two whitish teeth with smooth surfaces and edges.
Gums are pink, moist, and firm with tight margins to the tooth. No lesions or masses.
Bucal mucosa appears in pink color. Tissue is smooth and moist without lesions. Stenson’s ducts are visible with flow of saliva and with no redness, swelling,
pain, or moistness in area
Tongue appears pink, moist, and moderate in size with papillae and no lesions present.
The tongue’s ventral surface is smooth, shiny, and pink with visible veins and no lesions.
The frenulum is midline; Wharton’s ducts are visible, with moistness in the area. The client has no swelling, redness, or pain.
Sides of the toungue has no lesions, ulcers, or nodules also the tongue has a strong resistance.
The client can distinguish between sweet and salty.
The hard palate is pale or whitish with firm, transverse rugae
Palatine tissues are intact; soft palate is pinkish, movable, spongy, and smooth.
No unusual or foul odor is noted. The uvula hangs freely in the midline. No redness of or exudate from uvula or soft palate. Midline
elevation of uvula and symmetric elevation of the soft palate.
Tonsils are present, pink and symmetry in size.
Throat is normally pink, without exudate or
lesions
Neck Neck is symmetric, with head centered and without bulging masses.
The thyroid cartilage, cricoid cartilage move upward symmetrically as the client swallows.
Neck movement is smooth and controlled with 45-degree flexion, 55-degree extension, 40-degree lateral abduction, and 70-degree rotation.
Trachea is in midline.
Chest and Thorax normal
Abdomen
Upper Extremities Extremities are atraumatic in appearance without tenderness or deformity. Extremities are without swelling or erythema. Full range of motion is noted to all joints. Muscle strength is 5/5 bilaterally. Tendon function is normal. Capillary refill is less than 3 seconds in all extremities. Pulses palpable. Steady gait noted.
Lower Extremities Extremities are atraumatic in appearance without tenderness or deformity. Extremities are without swelling or erythema. Full range of motion is noted to all joints. Muscle strength is 5/5 bilaterally. Tendon function is normal. Capillary refill is less than 3 seconds in all extremities. Pulses palpable. Steady gait noted. [Show Less]
