It really enjoyed reading and learning about TransCultural Nursing (TCN) and the many individuals that have contributed to this fascinating subject. The following TCN scholars and leaders have enhanced, expanded, and advanced the specialty through their research, teaching, publications, conceptual models, frameworks, and/or theories: Josepha Campinha-Bacote (Campinha-Bacote,2011), Geri-Ann Galanti (Gilanti, 2014), Joyce Newman Giger (Giger, 2013), Marianne Jeffreys (Jeffreys & Dogan, 2014), Larry Purnell (Purnell, 2014), Marilyn Ray (Davidson & Ray, 2011; Ray, 2010; Ray, Turkel, & Cohn, 2011), Priscilla Sagar (Sagar, 2012, 2014, 2015), Rachel Spector (Spector, 2013), and the late Ruth Davidhizar (Andrews & Boyle, 2016, p. 9).
The model that I would apply to my current practice setting would be the Purnell Model for Cultural Competence. The Purnell Model has been classified by three well known nurse theorists as holographic and complexity theory because it includes a model and organizing framework that can be used by all healthcare providers in various disciplines and settings.(The Journal of Multicultural Nursing & Health 11:2). This model can assist providers to become more aware of the ethnic and racial issues and assist in enabling providers to provide better care that can be utilized in any healthcare setting. The model has 12 domains that sit in the center along with 4 outer rings that represent person, family, community and society. Each of these components having an effect on each other providing the provider with more insight on applying the appropriate care for that individual.
Working in a chronic dialysis setting you have the opportunity to see the same patients three to five times a week for three to 5 hours a day. Depending on where the clinic is located you can have a very diverse group of individuals. It is extremely important that our nurses and providers are culturally competent so that they can properly deliver the appropriate care to our renal patients. Many of our patients depending upon race and ethnicity have language and communication barriers, low literacy and access to health care is not always accessible.

I enjoyed reading your post and the Professors response. I find it very interesting that she has never had a student speak to this model. I believe this model could be utilized only by individuals that are spiritually connected and are comfortable in speaking and sharing religious beliefs and practices that align with the patient’s culture. Being able to connect with the patient at this level and assist in prayer has a great benefit on the patient as it may increase a sense of wellbeing.

I enjoyed reading your post and do agree that it is so important for us as nurses along with providers, patient care technicians and the healthcare community to understand the different cultures so that we are able to address the important elements of a patients culture. This may include addressing important religious holidays if they are hospitalized during that time. I also believe it would be good to know if there are any religious ceremonies that the culture may have when one is dying or even when someone is giving birth. Nutrition alone is important as there may be diet restrictions for acute or ongoing care. We as healthcare professions deal with patients from all parts of the community/world. These individuals are to be treated equally and receive the best care that aligns with their culture.
You had mentioned our text stating the importance of verbal and nonverbal communications for transcultural nurses. I have found this to be very important and some cultures do not utilize these gestures and doing so could offend them. Another great reason why it is so important to understand the culture you are taking care of. I like to do a little research prior to receiving my patient (of course if time allows) so that I can apply what I have learn to the patient. This has helped me in my current field. [Show Less]
