Dr. Battle and Class,
Medication errors are the most common errors that can happen in the clinical setting. Luckily, I haven’t seen someone else doing this mistake, but many times I almost did it myself. I believe such mistakes can happen due to lack of communication with physicians, nurses, pharmacists, and other health professionals. It is important to establish and maintain strong partnerships with health care providers and patients to reduce medical errors from happening. The way I prevented myself from doing a medication error is that I double checked with my instructor and other nurse before giving the medication to the patient. Also, it is necessary to always tell the patient the medication you are about to give them because many times the patient knows more about the medication and the dose than the nurse!
Now electronic prescriptions are widely used in clinical setting and are shown to reduce medical errors significantly compared to the paper prescriptions. Electronic prescribing eliminates the problem with handwriting legibility which can cause misreading of drug name, dose, allergy information, etc. In case medication error happened and the patient was given wrong drug or dose, the best is to monitor the patient for any side effects and give antidote if needed.
