WHAT= nurses follow this provision (4) by using three checks and following the rights of safe
medication administration.
WHEN= anytime and anywhere medication therapy is needed.
WHERE= any environment
WHY= is characterized as nursing’s response to the changing needs of society to achieve positive
client outcomes in keeping with nursing’s obligation to society through the medication
administration process.
HOW=the ways, means, methods, and behaviors that nurses use to practice professionally and
safely administer medications.
Federal Regulations: The U.S. government protects the health of its citizens by ensuring that medications
are safe and effective. The U.S. Food and Drug Administration (FDA) enforces medication laws
related to safety, potency, and efficacy.
Seven Rights of Medication Administration
1. The Right Medication: Review the medication order, compare the health care provider’s
medication order to the MAR (or eMAR), and then use the MAR to prepare and administer
medication(s). Use the three checks system.
2. The Right Dose: Confirm appropriateness of dose using drug reference. Check dose and verify
math calculation or conversions with another nurse; prepare dose using standards of measurement
3. The Right Patient: Check the name on the order and the patient. Use at least 2 identifiers. Ask
patient to identify themselves.
4. The Right Route: Check the order and appropriateness of the route ordered. Confirm the patient
can take or receive the medication by the ordered route.
5. The Right Time: Check the frequency of the ordered medication and that you are giving the
ordered dose at the correct time. Confirm when the last dose was given, if applicable prior to
administration. assess for a conflict of pharmacokinetic and pharmacodynamic drug
properties prescribed
6. The Right Documentation: Immediately document after administering the medication per
agency policy. Make sure to document any pre-assessment data that is required of certain
medications. Documentation of an incident report does not go in the client’s chart. It is a separate
documentation process for healthcare quality improvement.
7. The Right Indication: Confirm the rationale for the ordered medication. Does the medication
correlate with the patient’s history?

MEDICATION RECONCILIATION
Medication reconciliation occurs when nurses, pharmacists, or healthcare providers compare the
medications that a client is currently taking with what the client should be taking, was taking at home, and
any newly prescribed medications. Throughout this process, any duplicate orders are removed, and any
omissions are resolved. This also serves as a double check for compatibility. Usually occurs with
transition of care from floor-floor, or facility-facility
