During this video I will:
Propose an evidence-based care plan to improve the safety and outcomes for a patient
based on the Villa Health Remote Collaboration on Evidence-Care scenario.
Explain the ways in which an evidence-based practice model was used to develop the
care plan.
Reflect on which evidence was most relevant and useful when making decisions
regarding the care plan.
Identify benefits and strategies to mitigate the challenges of interdisciplinary
collaboration to plan care within the context of a remote team.
I’ll make mention of authors from supporting literature.
To begin, I am going to review the Villa Health scenario with you. Caitlyn, a two-year old girl,
was readmitted to the hospital for pneumonia within a 6 month period. Through collaboration
regarding Caitlyn’s presenting symptoms and history, the pediatric nurse, Virginia Anderson, and
the physician, Dr. Copeland, were able to later diagnosis her with cystic fibrosis. With this
diagnosis made, Virginia, Dr. Copeland, and the hospital respiratory therapist, Rebecca Hugo, sat
down to further discuss Caitlyn’s case. This interdisciplinary communication helped to identify
multiple barriers in and possible solutions to providing the best patient-centered care to Caitlyn.
These barriers include:
Living in a remote area with limited healthcare access.
Concerns regarding the day-to-day treatment of Caitlyn’s respiratory status to prevent
impaired gas exchange and respiratory distress.
Social issues related to the separation of Caitlyn’s parents, potential financial concerns,
and the need for caregiver support.
Potential morbidities of cystic fibrosis, such as DIOS, and how they could prevent
unnecessary trips to the hospital for this.
Once the above barriers were identified, the team was able to tackle each concern separately, and
develop Caitlyn’s patient-centered care plan. Collaboration between the hospital staff and
community partners, such as Caitlyn’s pediatrician and local community social services, was
imperative to ensuring the success of this plan.
I will now propose an evidence based care plan to improve Caitlyn’s safety and outcomes.
Cystic fibrosis is a progressive respiratory disease which requires lifelong treatment to maintain
health. Caitlyn’s success in living with this condition is dependent upon an evidence based care
plan which improves her safety and outcomes. Her nursing care plan should include:
maintaining adequate oxygenation, preventing complications, identifying knowledge deficits,
and addressing social issues which may impact her care.
As mentioned previously, there were four major barriers addressed during her hospital stay, and I
will address each one separately using the evidence based practice model for staff nurses. Cited

in 2016, Canada explains the evidence based practice model for staff nurses focuses’ on a staff
nurses involvement in implementing evidence at the bedside, which in Caitlyn’s case is her
home, to make sound, evidence-based clinical decisions. This model allows nurses to use critical
thinking skills, which is essential in the care of patients with limited access to healthcare. Let’s
