this site,I would you say to the
supervisor as a HSE representative,
that we need to investigate all the
details involving the accident that
would create a solution that would
prevent the accident from
happening again in the further. I
would also let him know that every
accident is not a human error, I
would explain about the techniques
used such as the tree analysis and
create barriers that would prevent
this accident from happening again.
I would also get the supervisor
more involved in the safety
investigation process to help him
understand that it is more than
saying that the human error caused
this accident. I would also compare
company rules to federal rules and
if a problem lies between the two, I
would have to get my company to
be in compliance with the law and
also get a speed retard if speed
was the case to keep the machinery
from operating over a certain
speed.
