Advanced pharmacology Week 8 Discussion
During Margaret's examination, her uterus was found to be intact, indicating that
progesterone therapy could
... [Show More] be used. The World Health Organization classifies IUDs from
copper and progestin, sterilization, and progestin implants as the best methods for
progesterone therapy (WHO). Because the failure rate of contraceptive methods is lower in
older women, some older women may be willing to use methods that are not as fast and as
effective, like oral contraceptives or sexual dependent methods like diaphragms, condoms,
and emergency contraception (Satoskar et al., 2017). The risk of venous thromboembolism
does not appear to be increased by using only progestins. It is a safe option for women who
are at a higher risk of having cardiovascular issues due to medical comorbidities, age, or
obesity.
Margaret has a family history of diabetes and high blood pressure, putting her at a
higher risk. Women with a healthy uterus should avoid using estrogen-only products.
Smoking is one of the number one issues that negatively affect women with a healthy uterus.
Estrogen-containing contraceptives are not recommended for women over the age of 35 who
smoke (Woo & Robinson, 2015). The doctor attending to Margaret should advise her against
smoking by explaining the negative effects. In patients taking estrogen, smoking increases the
risk of thromboembolic events.
Weight gain is linked to progesterone therapy. Patients older than 35 who smoke have
a higher risk of morbidity and mortality when taking progesterone. Margaret should be
pushed to exercise on a regular basis and be keen on her diet too. By maintaining exercise
and a good diet, Margaret has a better chance of remaining healthy even as her age
progresses. Margaret should be encouraged to quit smoking after learning that smoking
increases morbidity and mortality. Margaret could use a vaginal contraceptive ring or
intrauterine devices because her pelvic exam is normal (Yazidi, 2021). [Show Less]
