(PO 1) The Assignment What are some factors that make some people healthy and others ill? Healthy People 2020 identifies five determinants of health that influence the health of individuals and populations. Healthy People 2020 describes them as “a range of personal, social, economic, and environmental factors that influence health status” (U.S. Department of Health and Human Services, 2018, About Determinants of Health, para. 1). Determinants fall into five categories, including (a) policy making, (b) social factors, (c) health services, (d) individual behavior, and (e) biology and genetics. Choose one of the five determinants other than individual behavior (as we are all aware of how individual behavior impacts health). In your post, describe this determinant and its importance. (2014) Healthy People 2020 About Determinants of Health. ANSWER Professor and Class, Choose one of the five determinants and discuss its importance. This was a difficult decision as each of the five determinants of health are so closely interrelated, however I have discovered that policy making has the most widely influence on both individual and population health. In fact, policy making significantly impacts how successful and unsuccessful the additional four determinants can have on the vast majority of health outcomes. For example, it has been estimated that individual behaviors are responsible for approximately 50% of all premature deaths in the United States (McEwen & Nies, 2015). According to Healthy People 2020, tobacco use is classified under the determinant, individual behavior. Policy makers at the state level have been historically known to increase taxes on tobacco sales in efforts to reduce the number of smokers thus improving the overall population health (Healthy People 2020). Secondly, health services is by no means a stranger to the political realm as it continues to be among the top contenders of controversial debates. Fortunately, several literature have supported the correlation of access to health services and disease/illness prevention and health promotion. It was policy making efforts that enacted and signed into law The Patient Protection and Affordable Care Act on March 23, 2010. This law was created to improve access to affordable, quality health insurance for all Americans (Healthcare.gov). Still not convinced? Public safety, transportation options, and quality schools all fall under the umbrella of social determinants of health (Healthy People 2020). Unfortunately the longevity of these functions rely on funding received from the federal, state, and local levels. It was most recently in April 2018 that many marched to our Kentucky capital, Frankfort, to protest against further budget cuts towards public education during the state budget process of 2018-2020. This made national headlines ( Beam & Schreiner, 2018)! Discuss how a community nurse could use one of the 17 intervention categories from the Public Health Intervention Wheel to address this determinant. I have chosen advocacy for one important reason, it takes those to advocate for change to be heard at the government level! We, as professional nurses, should advocate for the general population, community, and/or individuals at the government level for an increase or preservation of current funding towards the prevention and promotion of health. This may include factors such as continued support of public education, cleaner water sanitation, continued affordable healthcare, and academic research. ANSWER While millions of people have gained coverage through the expansion of Medicaid under the Affordable Care Act (ACA), state decisions not to implement the expansion leave many without an affordable coverage option. Under the ACA, Medicaid eligibility is extended to nearly all low-income individuals with incomes at or below 138 percent of poverty ($28,676 for a family of three in 20181). The characteristics of the population that falls into the coverage gap largely mirror those of poor uninsured adults. For example, because racial/ethnic minorities are more likely than White non-Hispanics to lack insurance coverage and are more likely to live in families with low incomes, they are disproportionately represented among poor uninsured adults and among people in the coverage gap. Nationally, more than two million poor uninsured adults fall into the “coverage gap” that results from state decisions not to expand Medicaid, meaning their income is above current Medicaid eligibility but below the lower limit for Marketplace premium tax credits. These individuals would be eligible for Medicaid had their state chosen to expand coverage. As a nurse, I think adults in the coverage gap are likely to face barriers in health services or if they do require medical care, potentially serious financial consequences. Many people who are in fair or poor health or are in the age range when health problems start to arise but lack of coverage may lead them to postpone needed care due to the cost. While the safety net of clinics and hospitals that have traditionally served the uninsured population will continue to be an important source of care for the remaining uninsured under the ACA, and this system has been stretched in recent years due to increasing demand and limited resources. One way Community Nurses can help close the insurance gap is to participate in local health events that target the uninsured. Nurses need to be savvy about knowing which programs their own facility can and cannot provide to uninsured patients and to become familiar with their own processes for helping patients move between providers. Ground-level ozone is one of the biggest parts of smog. When ozone levels are above the national standard, everyone should try to limit their contact with it by reducing the amount of time spent outside. Air pollution is a leading environmental threat to human health. Particles in the air like dust, dirt, soot, and smoke are one kind of air pollution called particulate matter. Fine particulate matter, or PM2.5, is so small that it cannot be seen in the air. Breathing in PM2.5 may • lead to breathing problems, • make asthma symptoms or some heart conditions worse, and • lead to low birth weight. The national standard for annual PM2.5 levels is 12.0µg/m3. When PM2.5 levels are above 12, this means that air quality is more likely to affect your health. In 2014, the annual level of PM2.5 in Essex County was 9.9µg/m3. Elevated ozone concentrations are also damaging to crops and other plant life. It interferes with the ability of plants to produce and store food, making them more susceptible to disease, insects, other pollutants and harsh weather. Ground-level ozone damages the foliage of trees and other plants, ruining the landscape of cities, parks, forests, and recreation areas. ANSWER
For this week’s topic of discussion, I chose to talk about the setting and role of the Community Health Nurse in the school health setting, as well as an objective from Healthy People 2020 regarding that of bullying among adolescents. At-risk health behaviors typical for teens among the school-aged group tend toward things like violence, irresponsible sexual activity, poor dietary intake, suicide, inadequate physical activity, reckless driving, and the use/abuse of drugs, alcohol, and tobacco (Nies & McEwan, 2019). Community Health Nurses who specifically care for this aggregate population see students for everything from simple medical complaints to immunizations, mental health, health education, to learning disability screening (Nies & McEwan, 2019). Nurses, in this setting, function as a bridge between home and their health care provider (Nies & McEwan, 2019). School Nurses can also administer daily medications, perform vision tests and hearing tests, provide first aid, and provide parents with resources within their community (Nies & McEwan, 2019). NASN (2011) defines the role of the school nurse as “a specialized practice of professional nursing that advances the well-being, academic success, and lifelong achievement and health of students.” School Nurses also “facilitate positive student responses to normal development; promote health and safety, including a healthy environment; intervene with actual and potential health problems; provide case management services; and actively collaborate with others to build student and family capacity for adaptation, self-management, self-advocacy, and learning” (Nies & McEwan, 2019).
One of the most common issues related to a school-aged adolescent is stress. Stress can lead to more serious symptoms such as eating disorders, substance abuse, anger issues, class absenteeism, problems sleeping, physical exacerbations, sexual promiscuity, depression, and even suicidal ideations (Nies & McEwan, 2019). Depressive disorders usually begin in school-age years and have a direct correlation with the risk of suicidal behaviors (Nies & McEwan, 2019). As a School Nurse, it is especially imperative to be fully aware of typical warning signs and students in need of a mental health referral to a professional (Nies & McEwan, 2019). One such cause of depression can be related to violence, which is a significant risk factor affecting a student’s physical and psychological health (Nies & McEwan, 2019). Warning signs of Violence come in many forms indicating neglect or abuse (e.g. physical, emotional, or sexual) and can ultimately lead to criminal behaviors in some (Nies & McEwan, 2019). In the past decade, many of us have seen the effects of violence among adolescents in the form of school shootings.
A specific issue of interest that I keep hearing about in recent years, but has always been a problem, is bullying and its more recent partner-in-crime trend, cyber-bullying. The healthy objective topic I chose to address this issue, as a community health nurse relates to Violence Prevention – IVP-35 Reduce bullying among adolescents (USDHH, 2016). According to this data, nearly 20% of all high-school aged adolescents report they were bullied at school in 2009 (USDHH, 2016). While some studies suggest that bullying may stem from abuse during an adolescent’s past childhood, one study collected data to find the correlation between bullying behavior and abuse from current adult family members (Fujikawa et al., 2016). According to this study, more students identified themselves as victims of abuse from current adult family members than students who were not exposed to abuse from current adult family members (Fujiwaka et al., 2016). Additionally, the study also revealed there was an increase of suicidal ideations among adolescents dealing with abuse at home (Fujiwaka et al., 2016).
There are many methods and resources that we as community health nurses can use to identify, assess, advocate, and intervene for students whom we suspect are victims of bullying. We can start by noticing common clues that indicate certain health risks, such as depression, that can lead to bullying (Loftus, 2013). It is also important to be aware of common victims, which can be LGBT teens, ethnic minorities, teens with learning disabilities, and teens with physical limitations (Loftus, 2013). Antidiscrimination and anti-harassment policies can be put in place that include LGBT and ethnic diversities to help establish a societal norm, which in turn can change a student’s perceptions (Loftus, 2013). We as community health school nurses must also make sure we utilize specific interventional strategies to identify the root of the issues (Loftus, 2013). Additionally, these policies must be consistently implemented to foster a supportive and protected environment (Loftus, 2013). It is a good idea to raise awareness and gather input from teachers as well as other classmates who are more likely to witness such behavior. As school nurses, we are in a unique position to gain trust with adolescents. Students know we will not be handing out a grade and remain neutral. While bullying has and will always be a natural course of nature for some, there are still ways to end this cycle. Nurses working in the community are aware that there are different models for studying the epidemiology of a health condition in a population. One model of investigation of the interrelationships and characteristics of disease is the epidemiologic triangle. This model analyzes agent, host, an environment (Neis & McEwen, 2015, p. 70). By familiarizing oneself with statistical data developed through census and public health records, identification of corrective action plans can emerge. Different epidemiologic models were developed to study the many factors involved in the development of the health conditions plaguing communities –the web of causation model guides epidemiologists in researching factors associated with chronic illnesses (Neis & McEwen, 2015, p. 73). Investigative efforts often lead to identification of effective prevention measures –providing prevention for the community before has developed is a key initiative. As the Lesson (2015) discusses the value of studying epidemiological data to designing primary prevention efforts is paramount. Demographics Streetsboro, OH (Portage County) per U.S. Census Bureau, population in July 2010 was 16,028. According to U.S Census Bureau, as of 2010 Streetsboro, Portage County had 6,562 households, 31.4% of households had children of 18 years and below, 50.3% are married couples living in the same household, 11.1% of households have a female householder as the head with no other adult partner present, 34.2% are non-families, 26.5% are households with single individuals, and 6.8% of households has someone ages 65 years and above living alone. The average household size was 2.43 and the average family size was 2.97. The city’s population is spread out such that 22.3% were under the age of 18 years, 8.3% ranged from 18-24, 30.5 % from 25-44, 27% from 45-64, and 11.7% aged 65 years and above. The median age was 37.9 years. The gender makeup of the city was 48.5% male and 51.5% female. The median income for an average household in the city is $59,631, and that of a family $71,734. Males have a median income of $43,429 versus $24,854 for females. The city’s per capita income is $28,058. Approximately 5.83% of families and 8.5% of the total population lives below the poverty line. Epidemiology According to the 2016 Portage County health rankings, the priority health concerns for Portage County include- • Smoking - 20% of populace are adult smokers • Obesity - approximately 29% of adults report a BMI of 30 or greater. Although this ranking has remained the same it is above the national average. • Excessive drinking - 19% of adults report heavy drinking • Sexually transmitted infections - This measure is worsening at 221.1 newly reported cases of chlamydia per 100,000 population. • Primary Care Physicians - The ratio of population to physician is 2,410:1 • Mammography screening - 58% of female ages 67-69 enrolled in Medicare receive mammography screening. This ranking is worsening for the county and falls below the national and state average. • Severe Housing problems – 17% of households have at least 1 of 4 housing issues: overcrowding, high housing cost, or lack of kitchen or plumbing facilities. The Portage County Health Status Assessment data for 2015 was released on March 10, 2016. The data was obtained by independent researchers from the Toledo-based Hospital Council of Northwest Ohio (Portage County Community Health Assessment Partnership, 2016). Surveys taken consisted of customized questions and a set of core questions taken from the Center for Disease Control and Prevention’s Behavioral Risk Factor Surveillance System, Youth Risk Behavior Surveillance System, and National Survey for Children’s Health (PCCHAP). The survey was a cross-sectional, randomized sample of Portage County residents as follows: adults aged 19 years and older, youth aged 12 to 18 years, and parents of children aged 0 to 11 years. The Health Assessment data identified that 10% of Portage County adults were without health care coverage, the least likely to be uninsured were adults under age 30 (PCCHAP). In Portage County, 18.1% of residents live below the poverty level (PCCHAP). 22% of adults reported they were unable to access a physician within the past year due to cost. This percentage increased to 45% for those under the age of 30 and 33% of those with incomes less than $25,000 (PCCHAP). According to the Portage County Health Status Assessment, adults are affected by the four known risk factors for heart disease, 38% adults had been diagnosed with high blood cholesterol, 29% had high blood pressure, 13% were smokers, and 28% were obese (2016). In 2015, 22% of adults admitted to consuming who drank had five or more drinks (for males) and 4 or more drinks (for females) on one occasion (binge drinking) in the past month (PCCHAP). The Health Assessment identified that according to the BMI by age that 15% of Portage County youth were obese and 15% were considered overweight in 2015 (PCCHAP). Only 13% of Portage County youth admitted to eating 5 or more servings of fruits and vegetables per day, 80% ate 1 to 4 servings of fruits and vegetables per day. About 11% youth reported they went to bed hungry at least one night per week because their family could not afford enough food. 12% of youth did not participate in at least 60 minutes of physical activity on any day in the past week. The CDC recommends that children and adolescents participate in at least 60 minutes of physical activity per day. The 2016 Health Assessment indicated that 26% of Portage County youth had tried cigarette smoking, 21% of current youth smokers smoked cigarettes daily, and 17% of youth used e-cigarettes. The Health Assessment results indicated that 47% of Portage County youth in grades 6-12 had drank at least one drink of alcohol, increasing to 71% of youth seventeen and older. The Health assessment identified that 11% of youth grades 6th-12th had used marijuana at least once in the past 30 days, increasing to 17% of those ages 17 and older, 7% of youth used medications that were not prescribed for them, increasing to 11% of those over the age of 17. (27%) of Portage County youth have had sexual intercourse, increasing to 55% of those ages 17 and over, 55% had multiple sexual partners, and 26% of youth participated in sexting. The Health Assessment results showed that youths exhibit behaviors that endangered their own personal safety or safety of others. 18% of Portage County 6th-12th grade youth had seriously considered attempting suicide in the past year, 27% of youth drivers texted while driving, 23% of youth had been involved in a physical fight in the past year, 43% of youth had been bullied in the past year, and 33% had been bullied on school property. Homelessness is a major health factor in Pinellas County with 20-30% suffering from mental health disorders. This exercise has truly opened my eyes to see more of my community than I had previously known and has invited fresh insight to my role as a nurse.
