Nursing Care Delivery Models
Hi Everyone,
Nursing care delivery models vary across specialties and across institutions. What delivery model do you utilize in your practice? In my practice as a high risk OB nurse, I use the primary care model (ANA, 2015). I am the primary nurse for the patients that I am responsible for. This type of nursing is a relationship-based, autonomous model. I do have collaboration with other nurses and disciplines, when needed, but I am the nurse the patient will see on a weekly basis and I am responsible for her care and communicating any needs to our pharmacists, nurse director, and her physician; when necessary. This type of nursing has many pros. They are as follows:
A strong therapeutic relationship is built with patient and nurse
Promotes nurse autonomy and decision making
Patients are usually more satisfied with care
One point of communication between nurse and patient
There are also some cons that I see. They are as follows:
I am the only one in the home. There is no one to call if I cannot get an IV stick or if I feel I need another opinion
This can be costly for the institution, requires an all RN staff with experience in the specialty
If you do have a conflict in personalities or care plan with a patient, this can make the continuation of care more difficult.
I look forward to your discussion of your care delivery models and what you would change to improve patient care!
