Hi Class,
I work in long term rehab care and with a significant portion of our patient population being older, bed bound, and less mobile, pressure ulcers can be a particular problem for us to deal with. We also have patients who are para and quadriplegics, and with their extremely limited mobility, pressure ulcers are also an area of serious concern. One strategy that our facility implements is repositioning the patient once every 2 hours during waking hours.
To start researching the topic of pressure ulcers, we first start with the formulation of the question. We'll use the PICOT framework suggested by Melnyk and Fineout-Overholt (Hood, 2018). But their process also stipulates a refined question. So in our case it's not enough to look for "pressure ulcer reduction". I know what our facility uses as our current technique to mitigate pressure ulcers, but if there are better techniques out there, we should expand our PICOT question to compare various pressure ulcer reduction methods. So our question then turns into:
"What are techniques (I) used to reduce pressure ulcers (O) in bed bound patients (P) as well as their effectiveness compared to techniques used by our facility (C)?"
Armed with that question, we now have what we need to start researching in CINAHL. Since we want to get research studies that compared various pressure ulcer reduction methods, I entered "comparison of pressure ulcer prevention strategies". At this point I have series of research papers to start looking into. In order to see if my initial keyword search was specific enough, I look through the list of returned articles. Those that look promising I will drill into and look at the abstracts. If it looks promising, I will add it to my folder for further reading. As I go through articles, I may come across additional keywords that I can use to refine my existing search. Again I will look at the returned articles, look at the abstracts, and if they contain information that relates to my original PICOT question, I will add them to my folder for later reading.
Once I am satisfied that I have enough material to examine, I will go through the ones that I have selected and see if there are other pressure ulcer mitigation techniques that compare favorably or better than the methods that we currently use. I can then bring these up in future staff meetings and see if it is feasible to implement.
While this discussion doesn't cover the actual research itself, I did come across a very interesting article by Tayyib and Coyer (2016) that specially went into comparing various techniques to manage pressure ulcers. It is eye opening as it covers subject that I would not have even thought of. The article is in the references section should it be of interest.
